Many people when faced with a life-limiting illness, wish to spend their remaining days at home. For some, it is important to not be in a hospital but to remain in familiar surroundings with people they know and love.
Cotswold Care Hospice provides a specialist nursing service, called Hospice at Home. It enables a person to be cared for at home. The service also supports carers and families.
Our team of Registered Nurses and healthcare assistants work closely with your GP and Community Nursing team. Hospice at Home provides overnight care (10pm to 7am) or for agreed times during the day. All our services are provided free of charge.
You can only be referred to the service by any healthcare professional involved in your care. You cannot refer yourself or someone you care for. The Community Nursing team will act as your point of contact and will keep us informed of your situation and care needs.
Once you have been assessed, the Community Nursing team will plan and co-ordinate your care with the Hospice at Home team and let you know the name of the Hospice at Home member of staff who will visit and their planned time of arrival.
The Hospice at Home nurse is there to help you be as comfortable as possible and to carry out nursing procedures. This may include personal care, toileting, adminstration of medication, monitoring and control of symptoms.
Messages for Hospice at Home should be communicated via your Community Nursing team through their usual number. If there is an unexpected change in your requirements outside normal working hours (after 4.30pm week days and Bank Holidays), then please contact your District Nurse on the out-of-hours number: 08454 220220.
